DoReTec in NJ or Sound Values in Columbus sell the Dyna quad caps
for about $25 each.

There should be a 47k ohm resistor connected between the signal
and ground on each of the two input connectors.

There are several sources for 500v quads, Antique Audio and Sound Values 
both have 500v X 4 you might also consider using SCR caps available from
Mike Percy, this would require some modifications however.
